How to play Chart Decoder

  1. Start with Bootcamp. Learn the Greek and Latin building blocks (prefixes, roots, suffixes) plus the Rx shorthand used in orders.
  2. Pick a note. Each specialty has an inpatient, outpatient, or emergency note. Highlighted terms are the ones you'll decode.
  3. Decode, term by term. Choose the meaning. Right on the first try = 10 pts. After using a hint = 5 pts. Three in a row = streak bonus.
  4. Earn stars. 90%+ = 3 stars · 70%+ = 2 · 50%+ = 1. At the end, you'll see the whole note translated into plain language.

Why are there blanks in the notes?

Research and teaching datasets are de-identified before you ever see them. Under the HIPAA Safe Harbor method, 18 kinds of identifiers are removed, including names, dates (except year), MRNs, phone numbers, addresses, and ages over 89. Every dataset marks removed text differently:

Context is everything

The same letters can mean different things in different specialties. RA is room air in the ED and rheumatoid arthritis in rheumatology. AG is anion gap in the ICU and anticipatory guidance in pediatrics. Watch for these repeats.
